Acknowledgement of risk & waiver

1. Acknowledgement of risk *

I understand that participation in martial arts activities — including training, sparring, grappling, throwing, striking drills and strength and conditioning — involves inherent risks. These may include:

  • Minor injuries such as bruises, sprains and strains
  • Serious injuries such as fractures, dislocations and concussion
  • In extremely rare cases, catastrophic injury or death

I understand that Jujitsu is a contact sport and that physical contact is an integral part of training and assessment.

2. Voluntary participation & medical fitness *

I confirm that my child is taking part in this trial session voluntarily, that they are physically fit and medically able to take part in martial arts training, and that I have disclosed any medical conditions, injuries, allergies or additional needs that could affect their safe participation.

3. Assumption of risk *

I understand and accept that my child takes part in this session at their own risk, and that the inherent risks of a contact sport cannot be removed by the organisers.

This does not affect any rights we have in law, including where injury is caused by negligence or by anything else that cannot lawfully be excluded.

4. Release of liability *

I release, waive and discharge the following from liability for any loss, damage or injury sustained during the trial session:

  • Great Britain Jujitsu
  • The session organisers and coaches
  • The venue providers
  • Officials, volunteers and affiliated staff

This release does not apply where the loss, damage or injury is caused by negligence, or by anything else that cannot lawfully be excluded or limited.

5. Conduct & safety *

I understand that:

  • Participants must follow all coaching instructions at all times
  • Protective equipment must be worn where instructed
  • Jewellery must be removed, long hair tied back and nails kept short
  • Unsafe behaviour may result in removal from the session

6. Agreement *

I confirm that I have read this document carefully, that I fully understand its contents and that I complete it voluntarily. I confirm that I have parental responsibility for this child, or permission from someone who does, and that the information I have given is correct to the best of my knowledge.
